  • Any past treatment records (including all MRI’s, CT’s, radiology reports or other imaging records & reports) that pertain to your pain
  • If you have never seen anyone for your pain, contact your primary care doctor and request that they send the clinic your medical records
• Ketamine infusion costs vary from patient to patient. The total cost will be discussed at your initial appointment with Omaha Pain Physicians.
• The clinic can provide a superbill that you may submit to your insurance carrier for consideration of reimbursement.
